How does Carbidopa -Levodopa work?
Levodopa is inactive itself. Levodopa is decarboxylated in peripheral tissues (mainly gut and liver) in to dopamine. Carbidopa is an extra cerebral dopa decarboxylase inhibitor. Carbidopa cannot penetrate blood brain barrier but Carbidopa decreases the systemic concentration of dopamine and facilitate increased amount of levodopa to enter blood brain barrier.

Guidelines before using Carbidopa -Levodopa
In patients who are on levodopa therapy, Sinemet should be taken only after 8 hours of discontinuing plain levodopa. Carbidopa -Levodopa can be taken with food or milk to avoid stomach upset. Sinemet can cause false results with urine glucose testing in diabetics. Therefore in diabetic patients who are on Carbidopa –Levodopa, blood glucose testing should be performed instead. Carbidopa -Levodopa should be used cautiously in patients with a medical history of glaucoma, history of skin cancer, breathing problems, heart disease, kidney disease, liver disease, endocrine disease, ulcers, depression, and blood disorders. Sinemet should be used during pregnancy only if clearly needed. Breast feeding is not recommended while taking Sinemet. Avoid high-protein diets and iron supplements or products containing iron (e.g., multivitamins with minerals) as they may prevent absorption of Carbidopa –Levodopa. Avoid taking MAO inhibitors (e.g., furazolidone, isocarboxazid, linezolid, moclobemide, phenelzine, procarbazine, tranylcypromine) within 2 weeks before, during, and after treatment with Sinemet.

What are the common dosages of Sinemet?
The usual daily maintenance dose of levodopa is 0.4-0.8g along with 75-100mg of carbidopa, given in 3-4 divided doses. Therapy is started with a low dose and suitable preparations are chosen according to the needs of individual patients, increasing the dose as required.
